The NSW Agency for Clinical Innovation, NSW Ministry of Health, is updating the model of care (MOC) for NSW Paediatric Rheumatology Services, including transition services. In this session you will hear about the MOC and current resources for transition and discuss what young people with JIA/CRDs need to successfully navigate their transfer to adult rheumatology.
